Your Path for Success

Together with our team of medical professionals, family involvement is crucial to the overall success of a loved one’s stay with us. At the outset, we work with you and your family to ensure the very best care from the time you enter our doors until it’s time to return home.

How It Works:

1Prior to admission, one of our admissions staff can provide you with a brief explanation of what will happen upon arrival. They will also set up a time to have an initial meeting.

2The initial meeting where, with you and your family, we learn important information about you and your needs and preferences. And we will provide a brief orientation to the amenities of the facility, to help you get the most out of your stay with us.

3You and your family, working with our interdisciplinary team of medical professionals, will design a Total Care Plan that works toward expediting your recovery.

4The Discharge Plan, an “exit strategy” designed to help you create a “road map” for returning home successfully and functioning at the highest level possible long into the future.

5Finally, the Outpatient Care Plan for those patients who are able to return home but still need to visit the facility periodically for therapy treatments or other outpatient care.

EXPERT CARE

Different needs require different care

Wound Care

The team is dedicated to the care of patients with surgical, pressure, and vascular wounds. This specialized team of professionals provides comprehensive inpatient services for those who require expert care for skin and wound healing. Components of the Wound Care/Skin Integrity Program include specialty dressings, wound vac therapy, specialized airflow mattresses, 7 days a week coverage by wound certified RN’s, weekly interdisciplinary rounds with a wound physician allowing our residents to get home faster and healthier.

Chronic Renal Failure

The multidisciplinary team manages patients with renal failure maintaining ongoing communication with outpatient hemodialysis centers. An interdisciplinary team of caregivers works closely to provide and care for this patient population by maintaining laboratory values within range and ongoing monitoring for disease-related complications. The facility also provides transportation for patients on hemodialysis.

Heart PARC

Gateway’s HeartPARC program is a cardiac recovery program for Skilled nursing facilities that includes training for managing chronic conditions and preventing recurrent hospitalizations. Our interdisciplinary team has special training to decrease the readmission rate for cardiac conditions, improve patient’s ability to manage their chronic conditions and improve their function safely in order for them to return home and then continue with formal Cardiac Rehabilitation under the direction of their cardiologist.

Rehabilitation (PT/OT/ST)

Our therapy department is skilled in Geriatric Rehabilitation and a wide variety of acute and chronic conditions including congestive heart failure, Post Myocardial Infarction, Parkinson’s, Traumatic Brain Injuries, hemiplegia, vertigo and frequent falls, post orthopedic surgeries including total knee, total hip and spine surgeries, fractures and non-weight bearing issues. We are able to provide extensive therapy services more than 3 hours daily depending on patient need and tolerance. We have 6days a week therapy services. We have a full time Speech Language Pathologist trained and certified to provide Ampcare treatment for swallowing disorders (electrical stimulation for dysphagia) and LSVT ( Big and Loud for Parkinson’s) certification to improve communications for those patients with Parkinson’s Disease. Gateway has a warm pool that is ideal for treating patients with chronic pain or bariatric conditions, non-weight bearing conditions or severe debility.

Memory Care

Our professionals can reduce the stress and anxiety of being in a new environment for either a short-term rehabilitation or a permanent residence. The advanced Integrated Cognitive Program is instrumental in formulating the best care for our patients living with Alzheimer’s Disease.

Our Integrated Cognitive Program combines a standardized functional cognitive assessment of the patient with a relevant inventory of their daily life details, to help us develop a more-complete understanding of each individual under our care. By matching these two data sets in a meaningful way, we can better tailor our individualized therapy programs and care to the patient’s specific needs.

Provider Oversight

Providing the best care possible!

It’s important to us that our patients, as well as their family members, feel loved, informed and secure during their stay. Quality of life is just as important as quality of care in a skilled nursing facility! We work hard to keep patients happy and comfortable, and patients have consistently shown greater motivation in achieving their goals as a result. If there’s a special need or request, the Team Member has authority to act upon it.

Gateway provides a PM&R physician who is an MD or a DO who is specially trained in assessing a patient’s overall function. This gives us an advantage with our rehab patients. They are able to identify and address barriers to rehabilitation and functional progress. This includes additional training in managing pain and relying on non-medication treatments for patient’s conditions. In addition the facility also has coverage 4-days a week with nurse practitioners and physician assistants allowing our patients to get home faster and healthier.
